HIB Reporting

Anyone who witnesses or experiences HIB must report it promptly. Reporting is the first step toward resolution and prevention. Below are the reporting forms that can be utilized by families/caregivers and staff.

Staff HIB Reporting Instructions (LEA 338 Form)

Kinnelon staff can now submit internal HIB reports directly through Realtime.
Follow the steps below to access and complete the form.

Log in to Realtime
Go to: https://secure.realtimesis.com/kinnelon
Click “Login with Google” and sign in using your Kinnelon Gmail account.
Open the HIB Module
Navigate to Departments → HIB→Actions → Add HIB Incident
Complete the 338 LEA Reporting Form
Fill out the required fields and submit.
Automatic Notification
Once submitted, the principal and HIB specialists for your school will be automatically alerted.

Family/Caregiver Reporting Form

Kinnelon Public Schools uses Realtime’s HIB module to securely collect and review all family/caregiver-submitted HIB reports. Please use the steps below to report an incident.

1. Access the Digital HIB 338 Reporting Form

2. Complete All Required Fields. Please provide as much detail as possible.

3. Submit the Form. Once submitted, your report is automatically sent to your school:
