REGISTRATION INFORMATION

At the time of registration, the parent/guardian must present the following:

1. Completed Registration Packet which includes:
   
   1. Registration form (PDF)
   2. Policy 5111- ELIGIBILITY OF RESIDENT/NONRESIDENT PUPILS
      
   3. Immunization Records
      
   4. Emergency information card
      
   5. Transcript / Records Request form (PDF)
      
   6. Computer Use form (PDF)
      
   7. Permission Slip for local trips
      
2. Proof of Residency: as per BOE policy #5111. (click here)
3. Transfer Card from previous school.
4. Legal, Original Birth Certificate (with notarized seal). This will be returned to you immediately: as per BOE policy #5320.

Additional information (if available):

1. Test scores.
2. Special Service records.
3. Medical information (latest physical exam, health records, any allergies, health concerns and any medication taken on a regular basis.
